The Community Care Partners of Greater Mecklenburg (CCPGM) network is comprised of all sizes and levels of practices ranging from privately owned practices to clinics belonging to large hospital systems.

The program is a network of over 120+ practices within Mecklenburg, Union and Anson counties. It is comprised of primary care providers (General practices, Internal medicine, Family practices, Pediatrics and OB/Gyn).

Locating a Specialist
Referrals for specialty care can be made to any specialists who accept Medicaid by the assigned primary care provider (PCP).
