Recipient Enrollment
The department of social services is responsible for enrolling recipients into CCPGM. Each eligible recipient chooses a primary care provider from a list of participating PCPs. They also educate enrollees on how to use the plan, emphasizing the need for regular preventive health visits.

  • Mecklenburg County residents may change provider by contacting Mecklenburg Department of Social Service at 704-353-1500.
  • Union County residents may change their provider by contacting Union County Department of Social Services at 704-296-4300 and ask for their assigned case worker.
  • Anson County residents may change their provider by contacting Anson County Department of Social Service at 704-694-9351 and ask for their assigned case worker.

CCPGM is a network of Primary Care Providers (PCP) who will help you manage your healthcare needs.

  • The PCP you are assigned is on your Medicaid card.
  • Get established with your assigned PCP IMMEDIATELY.
  • Take your current Medicaid card when seeking medical service.
  • Call your PCP before going to the Emergency Room unless it is an emergency.
  • Get your PCP's approval before seeking specialty care or seeing another doctor.
  • Go for regular preventive care such as Health Check, immunizations, checkups, mammography, cholesterol and diabetic screening.
  • Some services do not need PCP approval such as dental or mental health services. (The patient handout provided by DSS includes a complete list of exempt services...)
