CCPGM Overview
Community Care Partners of Greater Mecklenburg (CCPGM) is a network of over 110 providers in Mecklenburg, Union and Anson counties, and
a component of CCNC. The program pro-actively identifies at risk enrollees that can benefit from case
management services.
The CCPGM focus is to ensure enrollees develop a relationship with a
Medical Home.
Program Governance and Operations
Medical Management Committee: Chaired by the CCPGM Medical Director and comprised of physicians from participating network practices. The Committee
reviews outcomes and discusses process improvement and cost-containment initiatives.
Steering Committees: Comprised of community providers and leaders from participating agencies including administrators, physicians, social workers,
dieticians, nurses, public health, area social services, and hospital specialists. These committees review outcomes data and generate recommendations for the Medical
Management Committee.
Clinical Care Coordination Services
Places emphasis on early detection and health promotion, providing disease management through individual case management interventions.
Clinical Care Coordinators (CCCs) are assigned to each practice and work with existing community resources to facilitate access to services for enrollees and removal
of barriers to accessibility of health care. CCCs identify enrollees needing assistance, assess, educate, and intervene in collaboration with the PCP.
Quality Improvement
Chart reviews are conducted at regular intervals to monitor quality, allow for trending, and guide process improvement efforts based on national guidelines such
as the American Diabetes Association (ADA) and the National Institutes of Health (NIH) for guidelines on care of patients with Asthma.
